About this service

Specialist Support Coordination (Level 3) is for participants who require intensive coordination due to complex needs, high-risk circumstances, or significant barriers in their support environment. Our specialist coordinators have the experience to navigate multi-agency situations, crisis periods, and intricate service systems.

Able Aide works collaboratively with hospitals, justice and housing services, allied health teams, and families to stabilise supports and achieve sustainable outcomes.

When is specialist support coordination funded?
The NDIA funds Specialist Support Coordination when a participant's complexity requires specialist coordination beyond Level 2. This is assessed by the NDIA based on individual circumstances.
Can hospitals refer directly for specialist coordination?
Yes. Hospitals and discharge planners can refer participants to Able Aide at referrals@ableaide.com.au. We prioritise urgent discharge coordination enquiries.
How is specialist coordination different from Level 2?
Specialist coordination is higher intensity and designed for complex situations — such as multiple agencies, high risk, or significant barriers — requiring specialist skills and more frequent intervention.
