Referral process

  1. Submit your referral

    Complete the referral form below or email referrals@ableaide.com.au with participant details, requested services, and urgency level.

  2. Acknowledgement within one business day

    Our intake team acknowledges every referral within one business day and assigns it to the appropriate service line.

  3. Initial contact and eligibility check

    We contact the referrer and/or participant within 2 business days to confirm NDIS plan details, service availability, and next steps.

  4. Service agreement and onboarding

    Once confirmed, we complete service agreements, introduce key contacts, and commence supports according to the agreed timeline.

Referral FAQ

Who can refer a participant to Able Aide?
Support coordinators, plan managers, GPs, allied health professionals, hospitals, schools, support workers, and family members can refer participants. Participants may also self-refer.
What information do I need for a referral?
We need the participant's name, contact details, NDIS number (if available), requested services, location, and any urgency notes. Additional clinical or plan information helps us respond faster but is not always required for initial contact.
How quickly will Able Aide respond to my referral?
We acknowledge all referrals within one business day and aim to make initial contact within two business days. Urgent hospital discharge referrals are prioritised for same-day contact where possible.
Can I refer for multiple services at once?
Yes. You can select multiple services on the referral form. Our intake team will coordinate onboarding across service lines.
Does the participant need to have NDIS plan funding for the service?
Yes. Services are delivered in accordance with the participant's NDIS plan funding. If funding is not yet confirmed, we can still discuss options and assist with planning.
What if the participant is not in my local area?
Our primary service areas are Melbourne's western suburbs including Tarneit, Werribee, Point Cook, Hoppers Crossing, and Wyndham Vale. Contact us to confirm availability for other locations.
