NDIS Plan Review Guide

Step-by-step preparation for an NDIS plan review, including evidence, goals and practical tips for families and participants.

If you would like support tailored to your goals, you can talk to our team.

What is an NDIS plan review?

An NDIS plan review checks whether the current plan is meeting the participant’s needs and goals. Reviews can be scheduled or requested when circumstances change.

If you’re preparing for a review, it can help to start gathering evidence early and writing down day-to-day impacts.

What evidence is helpful?

Evidence does not need to be complicated. Clear examples and recent reports are often most useful.

Allied health reports

Therapy reports, assessments, and recommendations that connect to functional needs and goals.

Support observations

Notes on what support is required, what happens without support, and what’s changed over time.

Daily living impacts

Examples from routines: personal care, safety, participation, behaviour, sensory needs, and fatigue.

Also see our Evidence & Reports Checklist.

Setting clear, practical goals

Goals work best when they reflect what matters most to the participant and family. They can cover daily living, community participation, independence, wellbeing, communication, and maintaining safe routines.

  • Keep goals specific and meaningful
  • Link goals to the supports you actually need
  • Include safety, sustainability, and participation where relevant

How Able Aide can help around a review

We can support families by providing clear support summaries, communicating with coordinators, and maintaining continuity of care before and after a plan change.
